If they've got him on Dilaudid, it's most likely what's making him so incoherent and confused. Get them to take him off the Dilaudid and onto some other pain med. If he refractured the hip it depends on where he fractured it, most fractures of that nature involve a fracture of the proximal femur. The compression screw may have to come out and he may have to have an austin moore hip hemiarthroplasty, possibly with a wiring of the fracture site depending on how bad it is. An austin moore is a slightly different approach, more through the posterior of the upper hip, but it's a quick surgery.
